About Care Coordinators of Colorado
We are two Care Managers who met in 2003 while working as Senior Enrollment Representatives. We both moved into roles as Ongoing and Intake Care Managers for Home and Community Based programs for Medicaid individuals. Over the years, we have seen all types of situations; whether it was in the home, with family members, transitioning of care, diverse cultural backgrounds, and medical histories. Each individual's case was a new learning experience.

Our goal was to keep individuals safe and connected in their community with the care and support needed.
As we have transitioned into the private sector, our goals and values have remained the same.

Our mission is to relieve the stress and burdens of the aging population and their families by knowing that they have someone working alongside them who has the knowledge and expertise in aging care management.

Amy Hladik
CCMC and Aging Life Care Professional
I am a Colorado native; born and raised in Colorado Springs. I was fortunate to live with my grandparents nearby. I was an only grandchild, and I spent countless hours with them. I looked up to my grandparents for how they valued friends and family. They were the inspiration for the path I chose in my career.

I graduated from CSU with a degree in Human Development and Family Studies with a focus in Gerontology. I then moved to Denver and worked in the Medicaid services sector for 18 years. I still live in Denver with my husband and two kids. We love everything about living in Denver; the culture, sports, proximity to mountain fun, and most of all the seasons!
Several years ago, I lost my dad. Within three short weeks I experienced providing personal care in all capacities: finding home care, setting up hospice, and figuring out how to get Power of Attorney status for someone who could no longer speak. I felt like all the years in the healthcare field were in adequate for the tasks that I had at hand. I truly felt empathy for what families of aging loved ones have to endure every day.

Sarah Geisler
I was raised in Colorado and graduated with a degree in Human Services from Metropolitan State College of Denver. I have worked in the healthcare profession for over 20 years and my main focus has been working with seniors. I am married and have two daughters.
I was blessed to have shared a close relationship with my grandparents. As they started to age, I realized the need for care management and how important it was to have an advocate promoting the amount of services available for them within their community.
With family support, my experience, and home care services, they lived to be 96 and 92. I am blessed to have been involved in their later years in life. I am adamant about sharing the knowledge I have accumulated throughout the years and therefore, treat everyone that I work for as family.
