We assess each client’s personal circumstances to develop a customized plan of care. We take into account the client’s unique identity; including religious/spiritual beliefs, cultural background, sexual orientation, gender identity, and family structure.
As Aging Life Care Professionals, we assist clients in attaining their maximum functional potential. The individual’s independence is encouraged, while safety and security concerns are also addressed. Aging Life Care Professionals are able to address a broad range of issues related to the well-being of their client. We have extensive knowledge about the costs, quality, and availability of resources in their communities.
Many people are caring for someone with dementia or other chronic illnesses and others live hundreds of miles away from an aging parent. We will be the coordinator for you (or your loved one) through the full spectrum of needs; from staying in your (their) best physical shape, to making health care connections and in-home care solutions.

to limit isolation and foster socialization, as well as

Assess housing appropriateness: Create stability and safety
in the home environment or transition to new housing -
Transitions of care (hospital stays, housing changes, or
adjustments to home care)

Each individual is unique.
By understanding one's needs and preferences; personalized support can help them achieve their goals, overcome obstacles, and improve their overall well-being. The individual always has the choice and the control over their care.
Short and Longterm Goals
As Aging Life Care Professionals, we conduct a comprehensive in-home assessment, to understand one's physical, emotional, and social needs. We tailor a plan to meet specific needs and set goals to ensure long-term success. We will coach the client and family to achieve the best results.
Coordinating Care
We collaborate with professionals such as physicians, attorneys, conservators and financial planners. Communication at all levels is an important part of our process; we are available by phone, email, and text. We use Caretree software for case organization and communication.
